I think the tattoo that is commonly found on their shoulders is something that was discussed here a few years ago. If my memory serves me right, it was supposed to act as a lucky charm to bring you good fortune and may have spiritual roots. It was in no way supposed to indicate gender surgery according to the person who posted here back then.
It may be that guys who felt female went through a lot of mental and physical abuse in their earlier days until they came out as women in their adulthood and either did or did not have subsequent surgery. If they were fragile mentally from this, then it is possible that a greater percentage of them than the rest of the population felt the need to get this tattoo, giving them spiritual comfort and better fortune. So the net result might be that it is more commonly seen in the 'K' community than elsewhere but is not by itself proof of gender issues.
